At Simple Integrated Care, our GP services are delivered through a coordinated clinical team.
Residents are supported by general practitioners, nurse practitioners, registered nurses and administrative staff who work together to provide consistent, responsive and well-coordinated care.
This team-based approach ensures care is not reliant on a single clinician and supports continuity, communication and quality of care across the residential aged care home.

Principal GP
Dr Jay leads our clinical team with a strong commitment to aged care medicine.
With extensive experience in geriatric health, chronic disease management and palliative care, he brings both clinical expertise and compassion to every patient interaction.
Dr Jay is known for his thoughtful approach and dedication to continuity of care within residential aged care settings.

Our nursing team supports care coordination, follow-up and communication between clinicians, facility staff and families.
They help ensure care plans are implemented and reviewed in line with each resident’s needs.

Rebecca supports the coordination of care across our GP services, working closely with clinicians, residential aged care home staff and families.
She helps ensure communication is clear, care is organised and residents are supported throughout their care journey.
Mitch Fox
Managing Director
Mitch oversees the strategic direction and operations of Simple Integrated Care.
With a background in healthcare leadership, he is focused on building sustainable models of care that support residents, families and providers across the aged care sector.
Working as One Team
Our team works closely with residential aged care home staff, allied health providers and families to ensure care is coordinated and responsive.
Through structured communication, case conferencing and shared care planning, we aim to provide a consistent and supportive experience for residents and those involved in their care.
