Supporting Residents and Families

Simple Integrated Care provides GP services to residents living in residential aged care homes.

Our goal is to ensure residents receive consistent, well-coordinated medical care while giving families confidence that their loved ones are supported by a structured and responsive clinical team.

A Consistent GP Service

Residents under our care are supported by a dedicated GP service that includes regular visits to the residential aged care home.

​

Our team works to provide continuity of care, ensuring residents are known, understood and supported over time.

A Planned Approach to Care

Within the first six weeks of a resident entering a residential aged care home and registering with our service, we provide:

​

  • A comprehensive medical assessment

  • A personalised care plan

  • Coordination with facility staff and care teams

 

This approach ensures care is planned, proactive and tailored to each resident’s needs.

Access to Care When It’s Needed

We understand that medical concerns can arise outside of scheduled visits.

​

To support residents, we provide:

​

  • Telehealth access twice daily, seven days a week

  • Nurse Practitioner support for triage and clinical care

  • Access to additional GPs where required

 

This ensures residents can receive timely care even if their usual GP is not immediately available.

A Team-Based Approach

Our GP services are supported by a broader clinical team, including:

​

  • General Practitioners

  • Nurse Practitioners

  • Registered Nurses

  • Allied health providers and specialists

 

This team works together to ensure care is coordinated, responsive and consistent.

Coordinated Care

We work closely with residential aged care home staff and other healthcare providers involved in a resident’s care.

This includes:

​

  • Communication with facility teams

  • Coordination with allied health and specialist services

  • Participation in case conferencing

 

This ensures all aspects of care are aligned and communicated.

New Residents and Patient Forms

If you are a new resident at one of our supported residential aged care homes, or supporting a family member who has recently entered care, we ask that you complete our patient registration and consent forms.

These forms help us provide safe, coordinated and timely care from the outset.

Patient Forms

You can download or complete all required forms electronically, including:

​

  • New patient registration and health questionnaire

  • Medicare billing consent and assignment of benefit

  • Privacy notice and consent information

  • Authority to discuss medical information

  • Request for transfer of medical records

  • Family communication preferences and feedback

Bookings and Telehealth

For residents currently under our care, telehealth appointments and clinical reviews can be arranged through our team.

Telehealth access is available to support timely care when concerns arise between scheduled GP visits.

No Out-of-Pocket Costs

Our services are delivered using available Medicare and DVA funding, meaning there are no out-of-pocket costs for residents.

Focused on Residential Aged Care

Our GP services are dedicated to residents living in residential aged care homes.

At this stage, we do not provide services to patients living in the community or those receiving care through Support at Home (formerly Home Care Packages).

​

This focus allows us to deliver a consistent and structured model of care within the aged care setting.
