Planned and Proactive Care
Our model focuses on planned care rather than reactive care.
Within the first six weeks of a resident entering a residential aged care home and registering with our service, we provide:

A comprehensive medical assessment

A personalised care plan

Coordination with facility staff and care teams
This plan is co-designed with the residential aged care home and forms the foundation for ongoing care.
Residents are then supported through regular GP visits, with additional check-ins as required.
Responsive Care When It’s Needed
We recognise that care needs do not always align with scheduled visits.
To support this, we provide:

Telehealth access twice daily, seven days a week

Nurse Practitioner triage and clinical support

Access to additional GPs where required
If a resident’s usual GP is unavailable, another GP within our team can access the shared clinical record and provide care without delay.
This ensures timely and continuous access to care within the residential aged care home.
Coordinated Care Across Providers
Our model includes routine coordination with all relevant healthcare providers involved in a resident’s care.
